If you have been diagnosed with Cancer and are a resident of Jay County you qualify for financial help.

JCCS reimburses money that has been spent for Non-Insured Cancer related expenses such as wigs, supplies, prosthetics. We also cover gas expense going to doctor appointments and treatments. To apply we need the following information:

• Doctor’s Diagnosis is needed. This is on a prescription pad(only need the first time you apply).
• Your Name, Address, Phone number
• For gas reimbursement we need your mileage t0 and from your Doctor appointment or treatment. We have a mileage percentage we pay.
• Cancer related products: Such as wigs, prosthetics, etc - We need the receipt.

Reimbursement Schedule:

• 1st year Patient - Up to $2500.00
• 2nd year Patient - Up to $2000.00
• 3rd year Patient - Up to $1500.00
• 4th year Patient  - Up to $1000.00
• 5th year Patient - Up to $700.00
• 6th year Patient - Up to $500.00
